About the Role

We are now looking to appoint a permanent full time and fabulous Grade 2 Social Worker at Dandenong Hospital in the adult acute space. You will have opportunities for amazing professional development, career growth and be a part of an awesome and supportive social work team at Monash Health. We are a busy and dynamic team assisting patients and families by providing a professional service for patients referred across the acute and women’s and children’s setting. Our Social Work service includes Acute, general medicine, surgical and specialty areas such as Intensive Care, Emergency Department and Women’s and Children’s services. The successful applicants will have the opportunity to be a part of a busy, energetic and friendly Allied Health team.

As a Monash Health employee, you will have the ability to maintain a high level of communication with patients, visitors and colleagues enabling you to deliver exceptional care. As a member of the team you are integral to the Monash Health Social Work service delivery and consistently demonstrate our core values. You will be part of a flexible workforce with personalised support. You will be a member of a dynamic team with the opportunity to work in a setting assisting adult patient cohorts from general medicine, general surgery, Emergency department, Women’s and Children’s and other specialty areas as required. The focus is on psychosocial assessments, risk assessments, complex discharge planning and family support.

What you Bring:

You are a qualified and experienced Social Worker with a minimum of 4 years post graduate experience providing a strong focus on patient centred care
Through your hard working and enthusiastic nature you have the ability to adapt to changing settings and shifts during the business week and weekends or public holidays
You are a dynamic Allied Health professional who is passionate about making a positive contribution to the health of your patients,
You are highly motivated to perform at your best every day, demonstrating excellent communication and interpersonal skills
Your dedication to safe best practice and quality care
An unwavering commitment to delivering patient centred care
Possess a positive attitude to your work, compassion and the ability to work collaboratively and effectively in a team
Motivation to be accountable for and manage specific portfolios in our relentless pursuit of excellence
Experience in a hospital or other health setting is essential
What you Need:

To be considered for this important role you will ideally possess the following:

An AASW accredited Bachelor degree or Master’s Degree of Social Work that provides eligibility for membership of the Australian Association of Social Workers
Good organisational skills and a positive, enthusiastic attitude towards work
Experience in working with culturally and linguistically diverse communities
Able to work autonomously
A friendly, outgoing and reliable demeanour
Well-developed clinical assessment and complex discharge planning skills
A current Victorian drivers licence
