About the Role
As the Nurse Practitioner, you will function autonomously and collaboratively in an advanced clinical role, and provide assessment and management of residents using nursing knowledge and skills which may include but is not limited to the direct referral of residents to other health professionals, prescribing medications and ordering diagnostic investigations.

You will provide innovative and flexible care delivery that compliments other health care providers, including:
• Providing advanced clinical skills and assessment/intervention and consultation.
• Developing a ‘shared care’ partnership with GPs, contributing to care plans and working collaboratively with GP’s to co-manage residents.
• Contribute to broader goals of hospital avoidance and improvement of the resident journey through person-centered care planning.
• The delivery of pharmacological and non-pharmacological management strategies.
• Clinical assessment management for the resident .
• Provision of information with residents, carers and health practitioners relating to the diagnosis, treatment options and treatment methods;
• The development of clinical guidelines associated with the speciality of aged care;
• Provision of clinical supervision and mentoring.
• Case conferencing with residents and person responsible

Essential requirements
• Current registration and practicing certificate as a Nurse Practitioner with AHPRA
• At least 5 years’ experience working in aged care (preferred).
• Ability to autonomously assess, investigate and implement appropriate treatment for residents within an agreed scope of practice.

Please note: The successful candidate will need to undertake criminal record checks and provide evidence of influenza and covid vaccination.
